If your student has a medical condition (asthma, severe food allergies, diabetes, migraines...), the clinic MUST have a Health Treatment Plan in order to treat your student with ANY medications.
For parents of children with ASTHMA, please make sure that the clinic has an inhaler and an Asthma Action Plan signed by you and their medical provider.
For SEVERE ALLERGIES, the clinic must have an Allergy Action Plan signed by you and a medical provider. This must be given to the school before any Epi-pens or other medications are delivered.
If you would like MEDICINE to be kept in the clinic (pain, allergies, cough/congestion), you must provide a signed Medication Authorization form (only 1 medicine per page).

Attention PWCS Parents:
All preschool, kindergarten, and new to the county students are required to submit a Virginia School Entrance Health (Physical) Form and immunization record, completed by a licensed provider, prior to their first day of school. School entrance physicals must be completed within one year of entry. Students who were previously enrolled in a PWCS preschool program, through Headstart or child find, are also required to submit an updated physical that falls within the dates above. This is in accordance with the Virginia Department of Education school entrance requirements and is a requirement for both in-person and virtual schooling within the county. These records must be provided to the school nurse prior to beginning school. **Effective July 1, 2021** A minimum of 2 doses of Hepatitis A vaccine are required for school entry. The first dose should be administered at age 12 months or older.
